2008 Annual Report Instructions
2008 Annual Progress Report for HSTW Sites

Please Note:
Some versions of Netscape are not compatible with the Annual Report
System. If you experience difficulties, please use Internet Explorer.
Please do not enter symbols, such as %, &, $, #, etc., in "number" or "percentage"
fields.
Instructions/Tips for Completing the Online Annual Report
Please read the following instructions in full before continuing
on to the report.
Doing so should save time and make the online entry process go much smoother.
After you have finished reading the instructions, click "Continue to Login" at
the bottom of this page to enter the online system.
1. Use the paper copy as a guide.
- Sites can save time by using a paper version of the Annual Report to make sure they
have gathered all the necessary information before logging into the system.
Schools should use a process that involves a group including the principal,
teachers, guidance counselors and other staff to discuss questions
in the report. The school leader should take full and
accurate notes from the discussion before going online to submit the school's
report.
- Please click
here to download a paper copy of the Annual Report.
- NOTE: Throughout the Annual Report, one "credit" represents 120
classroom or contact hours, or the equivalent.
2. Know your site code.
- You will be required to enter your site code along with contact
information for your school.
- If you don't remember your site code or
don't know what it is, please click
here
for a listing of site codes by state.
- If you don't see your school listed, e-mail
HSTWAnnualReport@sreb.org or leave a message at (404) 962-9628.
- PLEASE MAKE SURE YOUR SITE CODE IS ACCURATE AS THIS IS THE
UNIQUE IDENTIFIER FOR YOUR SCHOOL!
3. Review your information as you go.
- Please review all the information you enter on each page before
clicking "Continue."
- If you make a mistake you can click the "back" button on your
Web browser and change your responses. You must click the "Continue"
button to submit any changes made. Clicking the "forward" button on
your Web browser will not result in new information being submitted.
- Once you have submitted the Annual Report you will not be able to log on
again and make any changes. If you need to change any information on your
report please e-mail
HSTWAnnualReport@sreb.org.
- You will be able to print all pages of your Annual Report after submitting
it. You do not need to print each page as you go.
4. Complete all required fields.
- Some fields, such as the contact information for your school, are required.
You will not be able to continue with the report until these fields are completed.
- Other fields require additional information based on a prior
response. For example, if you indicated that your school has a requirement that
a minimum number of books are read in each class, you will be asked to give the
number of books. Please make sure this information is as accurate as
possible.
- NOTE: You must click "Continue" in order to enter each page of
information. Failure to do so will result in your answers not being
recorded.
5. Call if you have questions. Someone will be
available to reply to you Monday through Friday from 8 a.m. to 4
p.m.
6. Don't wait until the last minute!
