Information on the Ninth-Grade Student Survey
The Ninth-Grade Student Survey is administered to a random sample of 100 ninth-grade students, or all ninth-grade
students at schools with fewer than 100 ninth-graders. The 2008 administration window is April 1-30, 2008.
The cost is $1,000 for a random sample of 100 or fewer students plus $10 per additional student. This fee includes
survey administration and a report of the results. Ninth-Grade Survey reports will be delivered to all participating
schools as PDF files on a CD-Rom in early fall 2008. These reports will provide results for all students, but will not
detail specific student groups’ results. Your school may also order composite reports for $200 each. Composite reports
are in addition to the school report that is provided and provide survey results for a particular group of students.
All orders must be submitted by March 28, 2008.
Ordering the 2008 Ninth-Grade Student Survey
The 2008 Ninth-Grade Student Survey may be ordered by accessing the
online order system. Using the online method,
your order will take approximately 10 minutes to complete. Orders may not be partially entered and completed at a later time.
You will receive an e-mail confirmation that your order has been placed. If you do not receive a confirmation, your order has
not been received. All orders must be submitted by March 28, 2008.
You may also print a copy of the Ninth-Grade Student Survey Order Form using Adobe Acrobat and fax the completed
form to Aaron Boyd at (404) 872-1477.
